Open Question: Why do excellent students get rejected from the Ivy Leagues?

My friend's life long dream is to become a well renown doctor, and go to a prestigious college/med school. He applied to Harvard, Stanford, Yale, John Hopkins, and Columbia with a 4.6 gpa all honors/ap, 2300 sat score, and he volunteers at a hospital, and soup kitchen.
